  • We begin with your concerns.
    Dr. Sanchez will review your dental and medical history and the reason for your visit.
  • You receive a comprehensive evaluation.
    Dr. Sanchez will examine your periodontal and oral health. Necessary diagnostic records are included in the new patient fee and are selected according to your individual needs.
  • We discuss the findings.
    Dr. Sanchez will explain what he sees and whether periodontal or implant treatment is recommended.
  • You leave with clear next steps.
    If treatment is recommended, a treatment coordinator will review the treatment fees and available payment options with you.
  • Complete your forms.
    You will receive a text or email link to complete your registration, review office policies and submit the appointment payment.
  • Send recent full-mouth X-rays.
    If they were taken within the past 12 months, ask your dentist to send them to frontdesk@periowestmn.com.
  • Bring identification.
    Please bring a photo ID and your dental insurance card, if applicable.
  • For a second opinion, bring a copy of the treatment plan you would like Dr. Sanchez to review.
  • For patients under 18, a parent or guardian who is involved in treatment and financial decisions should attend.
  • New patient examination fee: $380. Payment is due at the time of the visit and includes the diagnostic records needed for the initial evaluation.

How Insurance Works at PerioWest

Clear information about out-of-network benefits, payment and reimbursement.

Out-of-network does not automatically mean no benefits

Some PPO dental plans provide out-of-network benefits for periodontal specialty care, and the difference may be smaller than patients expect.

Coverage varies by plan. Some dental plans provide preventive benefits only and do not cover periodontal services. Medicare Advantage dental benefits also vary and may not include periodontal care.

  • Payment is made to PerioWest.
    The full office fee is due at the time of service.
  • We submit your dental claim.
    PerioWest files the claim with your insurance company on your behalf.
  • Your plan processes the claim.
    The carrier applies its allowable fees, deductibles, maximums, history and plan restrictions.
  • Available benefits are sent to you.
    If eligible, the carrier sends reimbursement directly to the subscriber.

What Our Office Can Tell You

Out-of-network benefits: Whether the plan reports that benefits are available.

Procedure-code coverage: Whether specific codes are listed as covered, if the carrier provides this information.

Reimbursement amount: The carrier determines it only after processing the claim.

Guarantee of payment: Information provided by an insurance representative is not a guarantee.

We do not have access to the carrier’s complete fee schedules, claim history or plan restrictions, so we cannot estimate reimbursement.
